Long-Term Care Nursing

The Health Care Center at Plymouth Place in La Grange Park offers a variety of essential services to older adults for both long-term and short-term nursing care, including specialized memory support.

Through careful assessment, monitoring and evaluation of their needs, we support each resident. Our restorative philosophy is apparent in our activities programming, which integrates nursing care and meaningful activities. Services are provided by a staff of Registered Nurses, Licensed Practical Nurses and social workers, Certified Nursing Assistants, registered dieticians and activities professionals. Our staff-to-resident ratios exceed those required by the Illinois Department of Public Health (IDPH).

Licensed nurses monitor each resident’s medical condition on an ongoing basis. They coordinate services with the resident’s own physician, provide medication delivery and management, and administer any treatments required. CNA's help residents accomplish activities of daily living (ADLs), thereby maintaining the highest level of independence possible; residents are encouraged to handle as many of their own ADLs as possible.

Short-term rehab services such as physical therapy, occupational therapy and speech therapy are available as needed, and are provided in the Health Care Center by licensed therapists.

Residents and families are invited to work with the interdisciplinary team in developing an individualized plan of care. This plan reflects the unique needs and characteristics of each person and coordinates the efforts of the team to improve each resident’s quality of life.
